Find forms you already signed

Every form you sign is saved to your account so you can always come back for a copy.

Steps​

  1. Open Account: on a phone, tap the person icon at the top right of My Day; on the web, choose Account on the sidebar.
  2. Under My School, tap My Signed Forms.

  3. Tap a form. Each row shows when you signed it and which child it was for.

  4. To keep a copy, tap the share icon (phone) or the download icon (web).

Tips​

  • You can also open a signed form from your child's School Records → Registration → Policy Book with View signed copy.

Troubleshooting​

  • The list is empty. "Forms you sign will appear here." You have not signed a form in Asilo yet.
