Fields are the boxes a signer fills in. You place them on the PDF pages in the form editor. A desktop or tablet screen makes precise placement easier.
Before you start
- Finish Open School Forms first.
Steps
- Open the form in the Form editor (tap it in School Forms, or create a new one).
- Scroll to the page where the field belongs. The strip of field chips stays pinned at the top.

The field strip pinned above the PDF pages - Drag a chip onto the page:
- Signature and Initials: the signer taps to place their adopted signature.
- Signer Name, Child Name, Date: filled in automatically when the form is signed.
- Text: the signer types an answer.
- Drag the field to move it. Drag the corner handle to resize it.

A placed Signature field with its resize handle - Tap a field to give it a label, make a text field optional, or delete it. See Change a field's label or make it optional.
- Tap Save.
Tips
- Child Name is not available on staff forms.
- Line the bottom edge of a field up with the line printed on the PDF.
- Tap the eye icon to preview exactly what signers will see.
Troubleshooting
- A field will not move. It belongs to the Simple mode signature page. Turn Simple mode off to place fields yourself.
