Use Simple mode for a quick signature page

If you don't see this

• It needs the Add School Forms permission. A director grants it under Manage School → Staff → Permissions.

Simple mode is the fastest way to make a PDF signable. It appends one page titled "Signatures" with everything a signer needs.

Before you start

• Finish Open School Forms first.

Steps

1. Open the form in the Form editor.
2. Under the form name, tap Simple mode to turn it on.

3. Scroll to the end. A new page holds the signature rows:
   • Family forms: Child's Name, Parent / Guardian Name, Signature, Date.
   • Staff forms: Name, Signature, Date.
4. Tap Save.

Tips

• Names and the date fill in automatically when the form is signed. The signer only taps the signature box.
• Simple mode fields cannot be moved, resized, or deleted one at a time. Tapping one shows a reminder. Turn Simple mode off to remove the page.
• You can combine Simple mode with fields you place elsewhere, such as Initials on each page of a handbook.
